IMPORTANT: Do not leave any item unanswered. Write "NA" if not applicable. Place this application form together with your credentials in a long-sized brown envelope.

General Admission Information for the Academic Year 2008-2009

1. Applicants must submit the following requirements for evaluation:

a. Original Transcript of Records of the last 3 ½ years of Baccalaureate program
b. Four copies of colored pictures (2x2)
c. Two Certified copies of Birth Certificate
d. Certificate of Good Moral Character from a former professor or dean
e. Two letters of recommendation from a former professor or dean.
f. Police Clearance
g. Medical Clearance
h. CBC (complete blood count)
i. Urinalysis
j. Chest X-Ray (PA view)

2. Applicants must have taken the National Medical Admission Test (NMAT) before June 2008.

3. Applicants must pay the application fee and secure an application form from the Dean’s Office.
